The PrEP Counselor responsibilities include completing the patient intake assessments, development of service plans, following up on RX prior authorization, monitoring treatment adherence and re-evaluation of the patient's needs/discharge and termination. Maintain PrEP documentation in charts and web databases, educates and consults with service providers and consumers concerning HIV disease prevention and treatment and the process of meeting consumers identified needs. Coordinate the patient component services with all providers of the Primary Care/PHC department. Participates in case conferences and clinical team meetings as needed. Use social media resources to promote the PrEP Program. Services as a liaison between the clinic and the community for services outlined.
