Provides 40 hours per week of phone coverage and care coordination for youth referred to LPBHES.
Provides clinical service consistent with best practices in empirically based assessment and care for children and families.
Demonstrates professional conduct which exemplifies the NASW Code of Ethics.
Participates in program related meetings including but not limited to: staff meetings clinical conferences community meetings and in-services.
Completes documentation within Lifechart according to established policies and procedures.
Carries out responsibilities relating to Department of Social work including but not limited to attending social work staff meetings meeting requirements for reappointment and privileging and participating in staff development activities.
Attends group clinical supervision as arranged by the Social Work Department manager and pursues professional development.
Attends individual supervision with Access Supervisor and/or Director of Access.
Observes all hospital policies rules and regulations and all policies and procedures of the Social Work Department.
May participate in research or other specialized activities as arranged with the Director of LPBHES
Other duties as assigned by Director of LPBHES or Director of Access. Other information: BASIC KNOWLEDGE:
BSW from an accredited school of Social Work.
Demonstrated knowledge and skills necessary to provide care to child and adolescent patients with consideration of human development stages and cultural patterns in each step of the care process.
EXPERIENCE:
Relevant case management experience required.
Demonstrated ability to interact professionally both on the phone and with colleagues.
INDEPENDENT ACTION:
Performs independently within department policies and practices. Refers specific complex problems to supervisor where clarification of departmental policies and procedures may be required.
