Department:ICE | CORE Value Based CareDepartment's Website: Summary of Job Duties:This position is responsible for providing continuous quality improvement, project management, performance oversight, and the quality outcomes of population health and all value-based care efforts for primary care clinic patients in the Primary Care Population Health Service Line (PCPHSL) under the direction of the Quality Director for Population Health and Value Based Care. The Population Health Improvement Manager (PHIM) interfaces with the multidisciplinary leadership team including the clinic's access, nursing, physician, and administrative leadership to ensure quality and performance metrics are clearly defined and necessary workflows, interventions, and standard work practices are in place and executed. Additional Information: Additional Information: The PHIM provides oversight of site visits, manages quality plans, coordinates with the PCPHSL to addresses Quality, Patient Experience, and Safety outcomes. PHIM will report and address ongoing performance projects to improve and hold gains, help inform best practices, and support educational efforts.
