Position Purpose:
The Social Worker- Community Living has the primary responsibility of supporting the Quality Assurance Specialist-Community Living to ensure quality services are provided and the level of care and dignity individuals in Community Living day and residential programs are maintained at optimal levels. This directly affects the image and reputation of VersAbility in the community and by extension, on the communities' perception of people with disabilities.

Primary Duties:

* Conduct pre-screenings and coordinate/facilitate admissions and discharges for all CL programs. Maintains waiting list and referral list for all CL services. May include facilitating Discharge meetings as needed.

* Prepare minutes for IDT, QIDP, 30-Day and Follow-Up Meetings.

* Establish and maintain process for recruitment of viable candidates for CL residential programs (with Marketing Department).

* Coordinates with DBHDS to complete VIDES during admission and RST referrals. Update VIDES annually.

* Submit and obtain Consent forms, Right to Appeal letters, and Satisfaction Surveys from individuals/families and/or guardians.

* Provide guidance and support for community integration efforts and HCBS Rights Compliance.

* Provides accurate and timely documentation as required for Medicaid Waiver, Medicare, Social Security, ICF, licensing, etc. meeting ICF-ID requirements. This will include annual social work histories and monthly, quarterly, and/or yearly reports.

* Coordinates with contracted and other providers as needed to ensure individual needs are met financially, physically, and socially.

* Serves as a liaison with families, friends and other service organizations, including coordinating monthly and annual IDT meetings, planning activities to support family/individuals engage more, monitoring home visits with families, etc.

* Read, interpret, implement and understand policies, licensures, regulations, program requirements and state of the art methodologies of the treatment of persons with developmental and other disabilities.

* Assists Quality Assurance to ensure residential and day programs are continually in compliance with the standards and regulations of the Department of Licensure, DBHDS, Medicaid Waiver, CMS/ICF-ID, Department of Health, CARF, HCBS, Department of Social Services and any other applicable bodies.

* Review policies and procedures regularly for compliance with regulations and assist with reviewing/revising documents as needed.

* Inspect day and residential programs for cleanliness, safe environment, and recording of proper and timely documentation.

* Develop and submit audit schedule monthly and maintain documentation of audits on file.

* Complete Scorecard data monthly and serve as back-up to submit information into Scorecard Reporting system.

* Assists with staff training when requested.

* Assists in the implementation of new programs and services as needed.

* Maintain files of residents financial and medical resources, assist Finance Dept. with facilitating insurance claims and correcting inaccurate medical billing.

* Complete annual recertification applications for Medicaid and Social Security.

* Assist with maintaining HUD applications and files.

* Assist in developing and monitoring implementation of Corrective Action Plans. Maintain follow-up audits on file.

* Assist with assuming back-up coverage in absence of Quality Assurance CL-Compliance.

* Assist with carrying caseload in absence of Support Coordinators.

* Other related duties as assigned

Qualifications/Skills Knowledge & Abilities:

Education: Master's degree in Social Work preferred with 3 years of expeience; or Bachelor's degree in Social Work with 5 years Social Work experience. Must be able to maintain CPR, CPI, first aid and medication administration certifications on an ongoing basis. Must have a safe driving record and a valid state driver's license

Experience: Three years' experience working with adults with developmental and other disabilities, experience obtaining financial and medical resources, experience interpreting and implementing state, federal, or local regulations, experience designing and implementing state, federal, or local regulations, experience designing and implementing programs, experience coordinating admissions/discharges, or other equivalent experience that would prepare for this position.
