Job Summary:

The Hospital at Home focused Care Coordinator will assess and identify qualifying patients utilizing defined social and clinical eligibility criteria for the Hospital at Home program. The RN Care Coordinator will coordinate the patient onboarding process into the Hospital at Home program and coordinate admission activities while facilitating communication with the Hospital at Home care team and support staff to ensure seamless orchestration of patient logistics, patient education, and clinical care. Provides quality care to patients through the implementation of the nursing process applied through a team nursing model while maintaining high quality and safety standards.

Position Responsibilities:

Assess, identify, and document qualifying patients for the Hospital at Home program through the use of pre-defined social and clinical eligibility criteria. Collaborate with a team of multi-disciplinary clinicians to facilitate the patient admission process into the program and communicate required patient logistics required for admission and ongoing clinical care.

Distribute and educate patient and family on the use of the telehealth tablet used for biometric data capture, patient symptom reporting, and telehealth visits, as well as, any other outlined equipment and supplies needed for patient care.

Adheres to policies and procedures of the organization, including HIPAA and Patient confidentially regulations.

Position Qualifications Required:

Required Experience:

Minimum of 2 years of acute care nursing experience required. Experience working in an emergency department setting preferred. Ability to function independently and understand complex care management.

Required Education:

Graduate of an accredited nursing program.

BSN preferred.

Training / Certification / Licensure:

Current State of New Jersey nursing license to practice as a registered nurse. Current CPR certification and BLS.
