Summary: Under the supervision of the Program Manager/Patient Access Supervisor performs a variety of duties across the continuum of care to support families with children with special needs. This support includes administrative assistance in system navigation problem solving coordination of care and providing general information to families. Responsibilities: Assists and supports patient access for patients and families with children with special needs as it relates to scheduling/confirming patient appointments mailing reminder letters and handling general patient information and correspondence. Works closely with the Patient Access Supervisor on registration and patient scheduling responsibilities to support daily operations provider patient schedules and patient flow. Ensuring all daily arrived patient appointments are closed in the electronic medical record system by days end. Coordinates with all the specialty services within the clinic to ensure completed parent/teacher forms and other correspondence that have been completed are scanned into the patient*s electronic medical record for continued care coordination by providers and clinical support staff. Coordinates with the Lead Pediatric Family Services Coordinator for the Cedar Family Center assisting with completed letters forms and correspondence and ensuring this information is scanned into the patient*s electronic medical record for continued care coordination by the Cedar team. Answers clinic phone handling patient questions and concerns or directing to the appropriate clinical team. Assists with other duties as assigned by the Program Manager and Patient Access Supervisor. Other information: BASIC KNOWLEDGE: High school diploma or other equivalent. Demonstrated knowledge and skills necessary to provide administrative support for the care coordination of children with special needs. Interpersonal skills to effectively interface with Hospital personnel and persons from outside the Hospital such as vendors patients etc. EXPERIENCE: Related experience in a work setting providing care to children with special needs. (Personal experience as a parent or family member of a child with special needs preferred). Work requires in addition to the skills normally obtained through High School familiarity with working in a healthcare setting. Experience should also include familiarity with personal computers and electronic medical records. WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS: Normal office environment with some walking to assign work and review performance of clerical staff. INDEPENDENT ACTION: Performs independently within the department*s policies and practices. Refers specific complex problems to the program manager when clarification of the departmental policies and procedure is required. SUPERVISORY RESPONSIBILITY: None.
