Key Position Details: Job Description: Principle Responsibilities Provide direct care for family medicine patients at Hastings, MN clinic. Review patient medical history while collecting, prioritizing, and synthesizing comprehensive data pertinent to the patient's health or situation. Evaluate, interpret, and coordinate information to make decisions and recommendations regarding patient care. Provide education, consultation or intervention to patients (treatment plan) and their families. Work collaboratively with patients to improve their health and well-being. Practice in a thoughtful manner including consideration for cultural diversity, individual autonomy and legal responsibilities. Actively participate in multidisciplinary team approach to case management. Function appropriately as part of care delivery team through efficient use of resources and skills. Chart in electronic format in accordance with licensure and scope. May provide care in an ambulatory clinic, inpatient or outpatient hospital setting and perform office procedures. May participate in on-call rotation. Utilize quality improvement tools to develop processes that promote continuous improvement in delivery of patient care, patient outcomes, or workflows. May participate in peer review and quality assurance. Focus on preventative care; women's health; chronic pain concerns; osteopathic manipulation treatments; and addiction treatment. Required Qualifications MD, DO degree or equivalent. Must be BE/BC in Family Medicine with ABMS or AOA. Must have or be immediately eligible for MN medical license. Must have certification in BLS Tier 1 - Basic Life Support - Multisource. Must have hands on experience creating and implementing quality improvement tools to develop processes that promote continuous improvement in delivery of patient care, patient outcomes, or workflows. Must have hands on experience focusing on preventative care; women's health; chronic pain concerns; osteopathic manipulation treatments; and addiction treatment.
