Who you are:

As a Community Health Worker for Johns Hopkins Health Plans, you are the conduit between our lines of business and the community-based organizations that focus on the health and wellness of current and potential members. You are extremely comfortable reaching out to members via telephone. You are an advocate by nature and are passionate about empowering our members to take control of their health outcomes by connecting them to needed community resources, educating them about various care management programs, and encouraging preventive screenings. You understand the importance of your role in driving STARS and HEDIS quality measures and the mission of the organization.

What you'll do:




  • Collaborate with an interdisciplinary team of Physicians, Nurses, Care Managers, Social Workers, Health Educators, and other Community Health Workers (CHWs) to improve member health outcomes.
  • Under minimal supervision, address the complex social determinants of health (SDOH) problems (e.g., food insecurity, transportation needs, housing needs, etc.) of members enrolled in programs by connecting them with needed resources.
  • Provide telephonic outreach to members regarding care gaps, coordination of services, and to enroll members in educational and/or community/governmental support programs.
  • Foster cohesive relationships with members and with community partners and members' providers.
  • May be active in community engagement activities to include working with community organizations, knowing community members and their health issues, and assisting with community activities.
  • May include work-related travel (to and from community site, meetings, clinics, etc.).



What you'll bring:




  • 2+ years' experience in community health or health care related disciplines required.
  • Community Health Worker certification strongly preferred.
  • Understanding of STARS and HEDIS quality measures preferred.
  • High school diploma or equivalent required. Must be able to speak, read and write in English.
  • Spanish speaking preferred.
  • Preferred resident of Baltimore region, Eastern Shore region, or National Capital region

Salary Range: Minimum 19.92 per hour - Maximum 32.89 per hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.

In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
