POSITION SUMMARY:The Advanced Practice Professional (APP) has the clinical skills required for coordination and comprehensive management of pulmonary/Incidental Lung Nodule Clinic patients, including working with a multi-disciplinary team of Physicians, and Nurse Navigators. The APP coordinates involvement of medical specialty teams necessary to develop a comprehensive plan for the patient and reviews the plan of care with the other members of the clinical team and participates in directing that care.

  • The APP will work in collaboration with Physicians and APP's in managing a large volume of incidental lung nodule patients, ensuring all are managed in a timely manner and that none are lost to follow up.
  • The APP will perform and interpret, at least to the point of recognizing deviations from normal, common radiologic, and other routine diagnostic procedures used to identify abnormal pulmonary nodules.
  • The APP will assist in the process of triaging pulmonary nodules as they are referred to the clinic to ensure high risk nodules are seen in a timely manner.
  • The APP will act as a resource to Nurse Navigators on the team.
  • The APP will Participate in seeing lung nodule patients in the OP clinic.
  • The APP Conducts thorough medical histories, performs complete physical examinations (where indicated), initiates appropriate lab, radiology tests or other special tests required for evaluation of pulmonary nodules, and scrutinizes imaging on new and former clinic patients to establish a record of the patient's current health status and to develop a working diagnosis and treatment plan.
  • The APP will provide information and answers routine questions of patients regarding their disease process.
  • The APP will help to support and educate patients regarding their disease, treatments, related drug and treatment side effects and hazards.
  • The APP will Assist in scheduling procedures and managing cytology results.
  • The APP will refer patients to specialists as appropriate for consultation or for specialized health resources or treatment. (Specifically Medical oncology, radiation oncology, thoracic surgery)
  • The APP will assist the nurse navigators in management of an incidental lung nodule dashboard to ensure timely follow up based on recommended guidelines for care.
  • The APP will participate in thoracic conference/ nodule conference weekly, collaborating with physicians, oncology, radiology, and other healthcare team members regularly to ensure continuity of care by serving as a liaison between patient and other members of the multi-disciplinary care team or with other specialty areas as necessary.

Performs physical assessments of the patient, reviews their medical history, and interprets diagnostic tests/results in order to provide effective treatment for the patient.
Conducts preventive health screenings based on the patient's age, history, and unique needs to provide proactive health care.
Educates patients and their family about specialty care medical conditions, treatment, and follow-up.
Maintains record of all patient care in order to ensure accurate patient history.
Performs specialty care medical procedures in accordance with departmental guidelines and clinical roles as necessary within inpatient and outpatient care settings.
Identifies ways to improve inpatient and outpatient specialty care by assessing, problem-solving, and evaluating clinical and system issues.
Performs other duties as assigned.

EDUCATION:
Required: Master Degree Nursing or Master Degree Physician Assistant studies. Physician Assistant employed at Cone Health prior to October 14, 2024 with a Bachelor Degree will have the Master Degree requirement waived provided they have maintained employment and PA licensure with Board Certification.

EXPERIENCE:
Required: None
Preferred: 3+ years

LICENSURE/CERTIFICATION/REGISTRY/LISTING:
Required:

Valid NC RN and Nurse Midwife or NP license OR NC PA license

Board Certified within 1 year of date of hire by either the National Commission on Certification of Physician Assistants (NCCPA) for PA's OR the American Nursing Credentialing Center (ANCC) or the National Certification Corporation (NCC) for NP's OR the American Midwifery Certification Board (AMCB) or The American Association of Nurse Practitioners (AANP) or Pediatric Nursing Certification Board (PNCB)

BLS (CPR)-American Red Cross or AHA Healthcare Provider
Neonatal Resuscitation Protocol (NRP) Certification required for Neonatal Advanced Practice Providers
IVC Certified within 90 days of employment for Behavorial Health Advance Practice Provider
