We are a home health and hospice agency providing services in patient homes in 22 counties throughout Wisconsin. Patient homes include private residences, assisted living facilities, memory care units, and resident care apartment complexes. Preceptor strives to provide high quality care through our multifaceted programs including home health, hospice, palliative care, and outpatient services. We have branch offices located in Germantown, Fond du Lac, Waukesha, Sheboygan, and Appleton. We are currently in need of a Full-Time Authorization Specialist/Administrative Assistant in our Fond du Lac office Home Health Care, Hospice, and/or Outpatient Therapy Experience Preferred.
