Scope of Position In cooperation with other pharmacists and pharmacy technicians, acts as a resource and provides answers to drug information questions for patients, family, pharmacists, nurses, physicians, medical students, undergraduate and graduate pharmacy and students and other associate hospital personnel. Approves new drug orders, which may include checking/verifying chemotherapy notes, informed consent and processing non-formulary requests. Screens for drug-drug, drug-herb/dietary supplement, drug-allergy, drug-food (in cooperation with the Dietetics Department), drug-disease state and drug-laboratory interactions. Participates in appropriate hospital and/or departmental committees relevant to practice area. Position Summary This position functions to coordinate all aspects of patients' pharmaceutical needs in the James Cancer Hospital outpatient areas. Duties include but are not limited to: medication order approval and distribution; assurance of appropriate dose, route, frequency and duration of therapy; patient medication profile review for drug-drug interactions, duplication of therapy, drug allergy interactions and appropriate regimens; coordination of patients' pharmaceutical needs with physicians and nurses. Patient population includes adult and geriatric patients. Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must be able to demonstrate medication dosing and identify pharmaceutical needs relative to the disease process and age. This position is also responsible for supervising technicians, undergraduate and graduate student teaching, and providing educational programs for hospital/clinic staff (physicians, nurses and pharmacy staff). Minimum Qualifications For Hire: Bachelor's Degree from an accredited college of pharmacy and six month internship in a hospital pharmacy or an equivalent combination of education and experience. Licensed or eligible for licensure in Ohio. On going: Performs professional, supervisory, and teaching functions outlined above. Continues to meet competency and licensure expectations of a staff pharmacist. |
