Primary Purpose
The Certified Diabetes Educator (CDE) coordinates the clinical teaching and learning aspects of the Diabetes Healthways (DH) program. This role focuses on patient and family education, individual and group counseling, and personalized diabetes management plans. The CDE works closely with physicians and treatment team members to ensure the best outcomes for patients of all ages.

Key Responsibilities

  • Coordinate and facilitate the provision of DH services for assigned patients.
  • Assess patient and family needs, providing education and counseling on diabetes management.
  • Lead group education sessions and individual counseling on nutrition, medication management, and lifestyle changes.
  • Act as a case manager, collaborating with physicians and other care team members to develop and implement patient treatment plans.
  • Serve as a resource for diabetes management to hospital staff, patients, and the community.
  • Participate in support group activities and provide leadership as needed.
  • Conduct hospital staff in-services and educational programs, including residency grand rounds.
  • Assist physicians in managing inpatient insulin protocols and transitioning patients to subcutaneous insulin.
  • Proactively market the diabetes program to physicians, nurses, and community organizations.

Minimum Job Requirements

  • Education: Bachelor's degree in Nursing or Dietetics/Food & Nutrition.
  • Licensure:
    • For Nursing degree holders: Current Georgia Registered Nurse license.
    • For Dietetics/Nutrition degree holders: Registry with the Commission on Dietetic Registration (RD/LD) and Georgia Board of Examiners of Licensed Dietitians license.
  • Experience: At least two (2) years of clinical experience, preferably in diabetes patient education and supervision.
  • Skills: Strong communication, case management, and clinical assessment skills.

Preferred Qualifications

  • Master's degree in a related field.

Physical Requirements

  • Ability to sit for prolonged periods, lift up to 10 lbs., and perform physical movements such as bending, squatting, and reaching.
  • Must have functional range of motion and strength to operate office equipment and assist with patient care.
