Description MaineHealth Behavioral Health For a limited time MaineHealth is offering up to$5,000 Sign on bonus for all eligible Case Managers! Current MaineHealth member employees are ineligible; former MaineHealth Members are ineligible until greater than 3 months separation from employment. Bonuses may be prorated based on schedule. ~ 20 Hours/week, Part-time, Day shift ~ Maine state licensure to practice as aLicensed Social Worker (LSW) preferred. Multi-lingual preferred. Located at the pediatric clinic in Maine Medical Center. Summary The Early Childhood Case Manager is responsible for addressing social determinants of health (SDOH) and other risk factors that impede healthy development. The position will focus primarily on children ages 0-5 and includes parent coaching and team consultation, with the goal of promoting early child mental health, building resilience in the parent/child relationships, as well as accomplishing goals identified by each family. Establishing connection to the mother in women's health prior to birth of child is part of the work. The role will ensure that service delivery practices are ethical, culturally competent, trauma informed, respectful of diversity and inclusion, welcoming and geared to custom service satisfaction. TheEarly Childhood Case Managerwill be compliant with regulatory, accreditation, contract and best practice standards.Early Childhood Case Manageractivities will be performed primarily in the assigned pediatric clinic and via remote connection as project teams determine based on needs and social distancing guidelines.
