The Lead Social Worker collaborates with patients, primary care, and behavioral health specialties to ensure patients receive quality, efficient, and cost-effective healthcare services. Assists with the development, coordination, and optimization of the social worker role to improve overall patient outcomes. Develops and monitors practices and procedures for the social workers as well as provides training and orientation to all new and current team members. Will assist and guide the team to perform resource management, provide patient psychosocial support and referrals to community-based organizations.

MINIMUM QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Master's Degree in Social Work.

2.LGSW, LCSW or LICSW certification in the state of West Virginia OR 10 years of inpatient Social Work experience with LSW certification.

EXPERIENCE:

1. Three (3) years of clinical experience.

For PA locations:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Master's Degree in Social Work or related field.

2. Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW).

EXPERIENCE:

1. Three (3) years of social work experience with two (2) years in a supervisory role.

CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position. 1. Provides guidance regarding psychosocial assessments of patients and families to identify emotional, social, and environment strengths and problems related to their diagnosis, illness, treatment and/or life situation.

2. Collaborates with all members of the multidisciplinary team to facilitate appropriate community resource assistance for designated caseload.

3. Routinely meets and interacts with Population Health Social Workers to assist in daily, essential functions of the team.

4. Assists team members to formulate, develop, and implement a comprehensive psychosocial treatment plan utilizing appropriate clinical social work treatments and interventions. Interventions may include crisis intervention, brief and long-term individual, marital, family and/or group therapies as well as grief and bereavement work.

5. Works with the multidisciplinary team to assist with screening, identification, diagnosis, management, and treatment of victims of abuse, neglect, and domestic violence, rape, etc. Provide consultative services to health care team members within scope of care definitions, as needed. Maintain a working knowledge of relevant medical/legal issues that impact on patient care, e.g., advance directives, child, and elder abuse

6. Assists and provides guidance in the development and maintenance of documentation, assessment, plan, interactions, and interventions.

7. Maintains a working knowledge of community agencies and resources and serves as a liaison between them and the healthcare system.

8. Collaborates for appropriate resource and financial management, which may include but is not limited to-financial assistance coordination/referrals, entitlement program coordination/referrals, or patient benefit coordination.

9. Stays current with social work methods, theories, and health care developments to provide quality care.

10. Attends and participates in staff, committee, department, and other administrative meetings.

11. May participate in development, implementation, evaluation and revision of clinical pathways and other tools.

12. May educate the multidisciplinary team and physicians about clinical pathways/protocols and managed care principles

13. Works with leadership to design, implement, and evaluate a centralized care model that optimizes value. Works with leadership to continuously evaluate process, identify problems, and propose process improvement strategies

14. Clinical performance improvement, outcome management and quality activities as it relates to social work.

15. Provides guidance to the team regarding patient behavioral health and social needs.

16. Acts as the subject matter expert to help optimize the existing social workers Will work with the team to accomplish the organization's social work related goals.

17. Provides oversight in expansion of existing programs and the development of new programs.
