Position Status: Full-time, Day Shift

Position Location: On-site in Woodbridge, VA

Standard Working Hours: 8:00AM to 4:30PM

Minimum Requirements:

  • Minimum of two years of Case Management Experience or Long-Term Care Experience Required

  • One of the following licenses or education is Required: RN, LPN or bachelor's degree in health & human services

  • LCSW or LPC licensure also acceptable for certain positions

  • Job Experience Required: Nursing, Social Work or Human Service Field

  • One year working with Medicaid or CCC Plus populations, licensed in the state of VA Required

Job Description Summary

The Behavioral Health Navigator serves as a vital resource for patients experiencing psychological distress in a hospital setting to include the Emergency Department, Medical Floors, and Behavioral Health Unit. This role is dedicated to guiding patients through the often-complex behavioral health care system, ensuring they receive the appropriate support and treatment they need.

Key responsibilities include:

  • Conducting initial assessments to determine patients' behavioral health needs and identifying appropriate treatment options.

  • Connecting patients with mental health providers, community resources, and support services.

  • Coordinating and scheduling follow-up appointments to ensure continuity of care.

  • Addressing potential barriers to treatment, such as financial constraints, lack of transportation, or insurance challenges, and working to resolve these obstacles.

  • Providing ongoing education and emotional support to patients and their families, helping them understand their treatment plans and available resources.

  • Collaborating closely with medical staff, social workers, case managers, and other healthcare professionals to facilitate comprehensive care.

  • Advocating for patients' mental health needs within the healthcare system and the community.

Through compassionate guidance and a patient-centered approach, the Behavioral Health Navigator plays a crucial role in improving access to mental health services, enhancing patient outcomes, and ensuring a seamless transition to appropriate care.

Job Summary

The Regional Transition Care Coordinator (TCC) is responsible for managing and supporting all transitional care and post-acute services to members in a geographical region. Care transitions include transitioning individuals from Nursing Facilities, hospitals, inpatient rehabilitation, or other institutional settings into the community, and assisting individuals who desire to remain in their community setting. The TCC will work in collaboration with the Care Coordinator, Interdisciplinary Care Team Meetings, and facilities to facilitate a safe and effective transition. The TCC will plan, coordinate, monitor, and evaluate options to meet the individual's needs prior to transition back out into the community-based setting. The TCC will ensure a smooth hand off is given to the receiving Care Coordinator assigned to the individual.

Requires a minimum of two (2) years of Case Management Experience or Long-Term Care Experience.
