POSITION SUMMARY

Seeking a Licensed Social Worker/Licensed Mental Health Counselor to work in a psychiatric rehab setting. Excellent opportunity for clinician looking for dynamic position outside of clinic setting with daytime hours and no weekends. The program provides individuals with severe and persistent mental health conditions a full range of comprehensive, recovery-oriented services, with the purpose of assisting them in overcoming barriers caused by their mental health condition, developmental disability or substance use disorder.

This role serves as a member of the clinical treatment team providing clinical counseling/symptom monitoring, relapse prevention, crisis intervention and skills training related to integration of behavioral and physical health.

ESSENTIAL RESPONSIBILITIES



  • Participate as member of the clinical team under the supervision of the VP of Vocational and Therapeutic Services and PROS Program Director


  • Provide, implement and coordinate comprehensive initial and ongoing psychosocial, substance abuse, psychiatric rehabilitation and risk assessments for participants in the program. Provide individual evidence-based psychotherapeutic interventions, to assess, monitor and ameliorate symptoms; to minimize the negative effects of symptoms, behaviors and environmental stressors related to mental health conditions on levels of functioning and community integration; to prevent relapse.


  • Collaborate with individual, collaterals and their multi-disciplinary clinical treatment team (i.e. psychiatrist, nurse practitioner, registered nurse, certified rehabilitation counselors, licensed social workers, etc.) in developing, reviewing and modifying an Individualized Recovery Plan that identifies the medically necessary services that will support the individual's goals and path to recovery.
  • Provide engagement services with the purpose of fostering an individual's commitment to enter into therapeutic relationships and participate in program services supportive of the individual's recovery. These services also include group psychosocial rehabilitation services designed to support individuals in restoring interpersonal, self-management and basic living skills; prevent relapse; manage symptoms related to behavioral health conditions; monitor/address physical health conditions; strengthen wellness and self-care practices and achieving person-centered goals.
  • Provide crisis intervention services with the purpose of de-escalating situations of acute distress which require immediate attention and/or interrupt or ameliorate a psychiatric crisis.
  • Provide targeted interventions necessary to address immediate risks such as relapse, hospitalization, loss of housing or involvement with the criminal justice system.
  • Participate in orientation/intake process for new individuals as well as be an active member of the PROS Admission/Discharge Committee
  • Coordinate the discharge planning process. Identify the resources and supports needed for transition to another program and make necessary referrals, linkages for treatment, rehabilitation and support services.
  • Maintain electronic case records in a timely manner and complies with OMH and Medicaid regulations and agency policies and procedures.

  • Maintain a work environment in a clean, organized, and safe manner. Conduct daily inspections of work areas to look for unsafe conditions
  • Use all required safety devices and personal protective equipment.
  • Ensure all accidents are reported to supervisor and Human Resources.
  • Be familiar with the Health & Safety Program and abide by all safety and health rules, work practices, and regulations.

Qualifications

REQUIRED QUALIFICATIONS

Education:



  • Master's Degree in Social Work, Mental Health Counseling or related field.



Experience:



  • Two years' experience in providing services to individuals recovering from mental health disorders qualified by credentials and training. This can include internships hours during attainment of Master's Degree.
  • Must have a favorable result from the federal/state exclusions background check as well as a background check facilitated through fingerprinting and mandated by NYS law Chapter 575.



Licenses / Certifications:



  • Valid NYS License: Licensed Master Social Worker (LMSW)/Licensed Certified Social Worker (LCSW)/Licensed Mental Health Counselor (LMHC)



WORKING CONDITIONS

  • Must have a valid NYS Driver's License.
