Description Lutheran Social Services of WI & Upper MI is currently seeking a Long-term Care Functional Screener to join our IRIS Consulting team in Steven's Point, WI. This position will function primarily out of Steven's Point, but may also occasionally venture into surrounding counties. This is a full-time role at 40 hours per week. Hours will be primarily Monday through Friday first shift, but require some flexibility to support the needs of the individuals supported with a strong commitment to enabling people to be in control of their lives. Functional Screeners performs professional work involving the administration of Long Term Care Functional Screens (LTC FS) to participants enrolled in the IRIS (Include, Respect, I- Self Direct) program, in order to establish a level of care and functional eligibility for all publicly funded long-term care programs serving adults in Wisconsin. Essential Duties and responsibilities: This list of duties and responsibilities is not all-inclusive and may be expanded to include other duties and responsibilities as identified by the ICA Team.

CERTIFICATES, LICENSES, REGISTRATIONS: The employee of this position must have a valid Driver's License and car - Motor vehicle check (MVR) with a satisfactory driving record required. Must pass a Caregiver Background Check according to program regulations and LSS best practice.
