Job Summary The Senior Manager, Population Health plays a pivotal role in our organization's mission to enhance the health and well-being of the employers we serve. This key position focuses on the development and execution of strategies to improve population health outcomes through data-driven initiatives and care coordination. Key responsibilities are (1) developing and maintaining departmental overview, programs, protocols, and processes; (2) ensuring NCQA standards are being followed within department; (3) managing clinical staff and overseeing managers/assistant managers in day-to-day operations for designated products and services under Communitas Care Management; (4) ensuring operational efficiencies to achieve production and revenue targets. - Strategic Planning: Lead the development, implementation, and monitoring of population health strategies to enhance employer member health. (10%)
- Client Management: Effectively manage client relationships and accountable for the specific employer program(s) performance, administration and operations (20%)
- Data Analysis: Utilize data analytics tools to identify health trends, assess outcomes, and inform decision-making for population health programs. This includes, but not limited to, employer Performance Guarantees (15%)
- Budget Management: Manage budget allocations for programs, optimizing resource utilization. (10%)
- Quality Improvement: Ensuring department adheres with NCQA standards (10%)
- Team Performance: Develop departmental goals, build high performing teams through hiring / termination, training, and development. This requires setting clear expectations, providing feedback and motivating employees to achieve goals. (15%)
- Innovation Management: Create, develop, and implement new ideas, products and programs within the organization (10%)
- Issue Management: Address and resolve concerns and issues presented by internal and external partners, and any escalated concerns/issues from clinical staff. Responsible for evaluating all current operations within their designated department(s) and determine potential efficiencies and implement appropriate changes. (10%)
Qualifications
- 6+ years related work experience.
- 3+ years direct supervisory/management experience
- Related Bachelor's degree required Nursing
- Current RN license required - Compact license required
Additional Preferred Qualifications
- Previous experience with Regulatory Compliance and/or URAC/NCQA accreditation is beneficial
- Previous Managerial level experience in a healthcare setting preferred
