About the unit

The Corewell Health Reed City Hospital Susan P. Wheatlake Regional Cancer Center is a comprehensive regional treatment facility bringing together the full range of services for patients and families on their cancer journey. The cancer center features a Wellness Center, which includes a library, boutique, gift shop and an integrative therapy room. Patients can borrow educational materials from the center and also receive massages and integrative therapy services. The center hosts support groups and educational sessions to assist patients on the road to recovery.

About Outpatient Clinics

Corewell Health operates over 300 outpatient clinics across Michigan, bringing a multitude of care options to the communities that we serve. There are over 40 different types of primary care and specialty practices, which allows those who work in these clinics the opportunity to learn and explore various interests or to specialize in one area.

Scope of work

Provides peer support and role modeling related to the improvement of life skills and health/wellness to individuals and their families during personal interactions including home visits, telephone calls, etc. Instructs individuals and their families about wellness, chronic disease prevention and self-management. Assists clients and their families with accessing appropriate community resources. Assists with the coordination of group support programs and provides ongoing contact. Cultural competency and shared-life experiences with client population are preferred. Must have reliable transportation and be able to drive to and from appointments.

Qualifications

  • High School or Equivalent required, Associate's degree preferred
  • Successful completion of the Paraprofessional CHW Training Program required within 180 days from hire date
  • 3 years of relevant experience including knowledge and use of community resources; social, health and public agencies and services preferred
  • Basic Life Support (BLS) AMA American Heart Association, or ARC American Red Cross required within the first 90 days
  • Valid Michigan Driver's License required upon hire
  • Valid State of Michigan Auto Insurance required upon hire
