Responsibilities

Forest View Hospital, located just seven miles southeast of downtown Grand Rapids, Michigan, is a private 108 bed psychiatric facility that serves children, adolescents and adults. We are licensed by the State of Michigan, fully accredited by The Joint Commission and we bring more than 45 years of experience to the evaluation, diagnosis and treatment of a wide range of behavioral health problems. As a part of our continued success and growth, we are seeking qualified applicants for a Care Coordinator.

The Care Coordinator interviews patients and their families coordinating plans, programs, and activities that meet their emotional and social needs. Provides crisis intervention, assisting patients in understanding the implications and complexities of life situations, relationships, and its impacts on lifestyle. Serves as a liaison between the hospital, governmental, and external supports.

Essential Job Responsibilities:

  • Conducts pre-admissions assessments to ensure criteria is met for hospitalization.
  • Performs psychosocial assessments and evaluates patient needs, identifying presenting problems.
  • Provides life skills training and intervention strategies for patients, families, and groups to restore capacity for social functioning.
  • Coordinates services, monitors, and evaluates the outcome measures against the patients' personal goals.
  • Maintains relationships with community resource providers, particularly emphasizing facilitation of the referral process and providing continuity of care.
  • Accurately completes case notes on a continual basis which are factual, up to date and descriptive. All case notes are entered into the required data system according to current policy.
  • Assists in discharge planning and post -hospitalization community supports.
  • Other duties as assigned or needed.

Qualifications

Education and Experience Requirements:

  • Bachelor's degree from an accredited college or university in Social Work or other related human service field and one year of relevant social services experience.
  • Excellent written and verbal communications skills.
  • Ability to multi-task and manage multiple priorities.
  • Ability to remain calm and professional under pressure.
  • Thorough computer skills including all Microsoft office products.
