Job Description

Pay: $17.69/hour

Work Schedule: Part - Time Temporary

This position is eligible for Paid Sick and Safe Time (PSST) under the Healthy Families and Workplaces Act. All employees may also make pre-tax or post-tax (Roth) contributions to our retirement plan - must be 18 years of age to participate.

JOB SUMMARY:

The Provider, Direct Care will implement and maintain a Rehabilitation program for all assigned consumers to achieve their individual goals to provide intensive support services to develop the abilities necessary to maximize their independent functioning in a home and community environment.

ESSENTIAL FUNCTIONS:

Direct Care Services Program:



  • The Provider will meet with potential new clients to interview as a potential provider. The Provider will complete all client program paperwork to include Individual Services and Support Program (ISSP) tracking, monthly timesheets, incident reports. All assigned paperwork must be turned in by required deadlines. The incumbent is responsible for providing services specified in the Individual's Plan and assist them with working towards their goals and documenting the progress. The incumbent will schedule all client medical and dental appointments and ensure all follow up is completed, this is also to include any emergency appointments that are required. Traveling is a requirement of this program in which the provider will be responsible for and there will be no compensation for this expense.


Client/Program Tracking and Reporting:



  • The Provider must complete all necessary client specific documentation, e.g., timesheets, log notes, ISSP tracking, appointment encounter forms, doctor's orders, lab orders and results, and incident reports on a monthly basis. It is critical that the incumbent maintain complete and accurate records and statistical data as required for all clients and programs. All required paperwork must be turned in according to their designated due dates.


Relationship Management:



  • The Provider will participate with in-service training sessions. The Provider will deal effectively and calmly with non-routine situations following accepted guidelines and perform other duties as required by the Program Coordinator. The provider will follow all guidelines set forth by state regulations and will ensure that quality communication occurs between them, the client, and Goodwill of Colorado Staff.
  • The Direct Care Provider (DCP)To perform direct service tasks outlined in the plan of care, such as; personal care, emotional support housekeeping, home management.


QUALIFICATIONS:

Education:



  • High School Diploma or equivalent preferred.


Experience:



  • Experience in related field or equivalent combination of training and experience in a social service, counseling, or rehabilitation setting, providing direct services to persons with physical, emotional and/or developmental disabilities.


Other:



  • A valid Colorado driver's license without a history of violations is required.
  • The incumbent must be able to write clear, concise, and accurate reports and plans, submit them in a timely fashion and possess excellent time management skills. It is preferred that the incumbent have excellent written and verbal communication skills and demonstrate the ability to interact with various types of internal and external customers.
  • The incumbent must have a working knowledge of community resources, train and be creative in motivating consumers toward individual success.
  • The incumbent must also conduct self in a professional manner and maintain a professional appearance that enhances the firm's reputation.
  • It is also preferred that the incumbent have the ability to relate effectively to disabled person, their parents and guardians and be empathetic toward their needs.
  • The incumbent must have the ability to work independently, cooperatively and to exercise effective planning.
  • The Provider must demonstrate flexibility within the framework and pressure of established deadlines.
