Specialty Nurse Navigator - Neurology/Oncology - La Jolla/San Diego

This is a full time 8-hour shift position where you will work 1 day a week at La Jolla, and 1 day a week at Prebys Mercy Campus in Hillcrest (San Diego).

Position is partially remote, and you will be onsite 2 days a week, 3 days a week remote.

In this role you will work with a multidisciplinary team navigating patients with a new cancer diagnosis. Bring your Neurosurgery, Neurology and/or experience caring for patients with brain tumors.

What will you do?

• Develop personalized care plans based on comprehensive assessments and clinical data, ensuring adherence to established clinical guidelines and standards of care.
• Set clear, measurable, and realistic nursing goals that are time-bound to effectively guide the care process and foster patient self-reliance.
• Support patients in building, adjusting, or enhancing their social support networks to further promote autonomy.
• Identify and connect resources critical to managing high-risk care.
• Coordinate services for high-risk patients, collaborating with the healthcare team to monitor and analyze data for research initiatives and resource utilization.
• Provide recommendations based on findings, particularly in relation to relevant specialties.

Required Education/Experience/Specialized Skills:

* BSN

* Three years recent clinical nursing experience in a variety of clinical settings.

Required Certification/Registration:

* BLS, CA RN License

Preferred Education/Experience/Specialized Skills/Certification:

* Neurosurgery, Neurology or experience with brain tumors experience highly preferred.

* OCN.

* Two years' experience managing care of relevant specialized patient population.
