Overview
Now hiring a Senior Program Manager for Community Health! Presbyterian is seeking a Senior Program Manager for Community Health. This position is solely responsible for managing the design, development, and implementation of community health initiatives to improve population health outcomes for our patients, members and communities. Accountable for collaboration and engagement methodologies with extensive community, public health, and population health relationships. This position will lead and participate in multi-disciplinary engagement teams which leverage integration to maximize results while also weighing the practical considerations and potential barriers that exist when implementing programs in clinical and diverse community settings. Ideal Candidate: Bachelors degree in public health; community health; public administration, education or related field required. Masters degree preferred, but not required based upon relevant experience. Seven (7) or more years of relevant industry specific experience demonstrating strong leadership skills and a proven ability to facilitate cross-functional teams.

- Seven (7) or more years of relevant industry specific experience demonstrating strong leadership skills and a proven ability to facilitate cross-functional teams.
- Experience facilitating diverse community-based groups to foster partnerships.
- Strong research competencies, written and oral communication.
- Computer knowledge to include Windows, word-processing and database systems.
- Requires detailed knowledge of population health and public health research and interventions, including the interpretation of data
- Data management and insight reporting, preferably experience with Electronic Health Records

Responsibilities
- Responsible for the coordination of Presbyterian community health initiatives.
- Promote and support adoption and implementation of an overall community health approach across PHS.
- Develops organizational capacity for innovation, development of best practices and knowledge transfer of these assets.
- Leads the development and deployment of strategies to align practitioners with new community and clinical linkage models designed to improve population health.
- Leads the development and deployment of strategies to engage providers and community in adoption of programs.
- Prepares relevant program reports Engages and influences internal and external (government entities and community organizations) stakeholders.
- Actively participates on relevant community councils and/or Boards representing Presbyterian
- Identifies community health program related grant funding and develops grant proposals and supports tracking and reporting on grant deliverables.
- Present grant deliverables to Presbyterian Senior Leaders.
- Manages specific community health programs from planning and design through implementation and evaluation.
- Identifies relevant health initiatives in the community for achieving collective impact goals identified through community health needs assessments and plans.
- Uses integrated decision making to ensure optimized outcomes.
- Coordinates and deploys communication strategies to internal and external stakeholders regarding community health initiatives.
- Helps establish, organize, and maintain program-related data management plans [data collection, datasets, databases- including EHRs, security, privacy retention, and reporting].
- Provides user and technical support on program data operation standards and tools, including Epic, Salesforce, closed-loop referral software, and others as needed.
- Gathers data, analyzes metrics, and coordinates the documentation, measurement and reporting internally and externally of initiatives.
- Provides research, analytical, information support, and recommendations when requested, for decision making.
- Position may include support for front line workers and in some cases may provide direct patient support.
