Job Purpose

The Transitional Housing Programs Lead Case Manager will support the operation and delivery of case management services at the Transitional Housing location to which they are assigned. The incumbent will monitor and support at least one other Case Manager and several House Managers. They will report to the Director of THP on activities, staff productivity, and completion of various team projects, along with carrying a case load of clients.

Qualifications

  • Female Only
  • MSW is preferred but a Bachelor's degree in Social Services is OK
  • Minimum two years of case management experience with at risk populations
  • Trained in Harm Reduction
  • Trained in Motivational Interviewing
  • Reliable personal automobile
  • Current valid driver's license and proof of current auto insurance
  • Certified in AODA Counseling is a plus
  • Experience working with veterans, persons with substance abuse and or mental health disorders is a plus
  • Knowledge of Clarity is a plus


Duties and Responsibilities

  • Conduct all intake and assessments for incoming clients (typically 6-10 per year)
  • Determine and document eligibility, provide admission procedures and program orientation
  • Maintain accurate and timely records of clients through detailed case noting of assessments, contacts and progress using required case management forms
  • Maintain the required Homeless Management Information System Records in Clarity
  • Develop and monitor a "Plan of Care" that establishes short and long term goals for the client that will be monitored and updated regularly
  • Make program referrals and establish linkages with appropriate agencies/Service providers to assist participants in obtaining needed supportive services
  • Assist parent(s) to secure child(rens) documents/records and connect to appropriate resources
  • Maintain a daily calendar of all activities in Microsoft Outlook
  • Properly manage stress and maintain a regiment of self-care
  • Other duties as assigned or requested
