Summary of Job Duties:We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) II to join our dynamic team as a Care Coordinator. This role is essential in ensuring seamless patient care transitions and supporting clinical staff in delivering high-quality healthcare services. The LPN Care Coordinator will work under supervision to meet the diverse needs of our patients, contributing to closing care gaps, managing medications, and providing administrative support. The position may at times require filling in for absent nurses or other clinic personnel, and/or performing assigned administrative or other patient care duties. This position must be able to perform all duties within the scope of the Licensed Practical Nurse and undertake additional duties to support patient care activities as needed. Qualifications:

Minimum Qualifications:

  • Graduate of an accredited Licensed Practical Nurse Program
  • Minimum of one (1) year of work experience in the ambulatory clinical environment with direct patient care.
  • Current active AR LPN license
  • Current CPR certification

Key Responsibilities

  • Works in collaboration with multidisciplinary provider teams to provide innovative and evidence-based care.
  • Works with the patient and the patient's care team to develop an individualized treatment care plan-including follow-up appointments.
  • Contacts patient between visits via telephone, MyChart(secure messaging), or letter to check on self-monitoring, provide encouragement and support, and assesses patient progress toward health status goals.
  • Assesses barriers when patient has not met treatments goals, is not following treatment plan of care, or has not kept important appointments.
  • Assist with addressing barriers, anticipating the needs of the patient panel, ensuring that ordered referrals and testing are scheduled and completed and that the necessary data and documentation is completed, retrieved, updated in the EMR, and/or requested prior to the patient visit.
  • Actively assist in needs for patient hospitalizations, making referrals appointments to specialists, for ancillary testing, and other services.
  • Educates patients about health preserving, disease prevention, and disease self-management interventions, creating a collaborative relationship with the patient in the management of their health care status.
  • Disseminates education materials and renders training to patients related to disease status or prevention. Document all interactions with the patient in the EMR phone note template.
  • Performs outreach to ensure that patients receive reminders of the need for preventive or disease management screening and testing, including point-of-care reminders. Schedules preventive care appointments (colon, cervical, breast, eye exams) when needed as part of outreach.
  • Assists in maintaining a community resource list of services available to patients and maintains professional relationships with community resource agencies used most frequently.
  • Refers patients to other entities for education and support as needed. (Behavioral Health Consultant, Pharmacist for Diabetes Education, Tobacco Treatment Specialist).
  • Answer incoming phone calls and return phone calls from patients, physician offices, and pharmacies as needed to address the need per protocol.
  • Sort and address incoming faxes (test results, consults, DME, prescriptions, FMLA, etc.) on the fax server. This requires that documents be reviewed and dispersed to the correct location or individual (care manager, scanning, importing, physician mailbox, EHR, etc.).
  • Fill in for other clinic personnel and/or perform assigned patient care duties as needed or assigned by direct supervisor.
  • Assists, as needed, with triage of urgent matters that occur in the clinical area.
  • May perform other duties as assigned
