Job Details Minimum Education and Experience Requirements:Education: Graduate of an accredited school of nursing required; BSN or completion of BSN or appropriate advanced degree within 3 years of hire strongly preferred. AND Experience: 1-2 years experience in similar role preferred but not required. Minimum Certifications, Registration or License Requirements: Current RN licensure in state of practice; BLS required based on organizational guidelines/polices Job Description The Nurse Navigator will contribute expertise as a member of a multi-disciplinary team to facilitate the provision of quality cost effective care for outpatients and their caregivers. The Nurse Navigator will provide ongoing patient management in the outpatient setting as directed by the physician. The Nurse Navigator will provide and coordinate outpatient education and resources to meet indicator outcomes. Will serve as an integrator between physician and other disciplines to expedite interventions which optimize clinical outcomes. -Performs chart review.
