Position Summary

The Lung Cancer Screening Coordinator is primarily responsible for the coordination of care and scheduling for the Lung Cancer Screening (LCS) program. Facilitates timely and effective care coordination for patients identified with lung nodules through the LCS program and AZRA AI.

Responsibilities

  • Care Coordination: Manage the tracking and follow-up process for patients identified with lung nodules through the LCS program, ensuring timely communication between providers, specialists, and patients.
  • Scheduling: Reviews referrals to the LCS clinic and schedules patients timely and accurately in the LCS clinic. Coordinates outgoing referrals from the LCS program.
  • Technology Utilization: Leverage technology and electronic health record (EHR) systems to monitor patient progress, update records, and generate reports for multidisciplinary teams.
  • Collaboration: Work closely with pulmonologists, apps, radiologists, surgeons, oncologists, and administrative staff to streamline workflows and remove barriers to timely care.
  • Patient Outreach & Support: Act as a liaison between patients and providers, ensuring patients understand their next steps and feel supported throughout their care journey.
  • Data & Reporting: Maintain accurate records, track patient outcomes, and assist in developing quality improvement initiatives related to lung cancer screening and nodule management.
  • Process Improvement: Identify gaps or delays in care coordination and proactively work with stakeholders to implement solutions.

Requirements

  • High school graduate or equivalent required
  • CMA/LPN/Rad Tech highly preferred
  • Two years of clerical experience in a medical environment
  • Experience in healthcare coordination, patient navigation, or a related administrative role

Pay and Benefit Expectations

We provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $22.97 - $33.05 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education. If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicant's years of experience align.
