Clinical Social Worker 1
Job Summary Within a multidisciplinary treatment team as part of the Family Health Center, Bridge Clinic located at Zuckerberg San Francisco General Hospital, the Clinical Social Worker I/II (CSW I/II) will work with people who use drugs and alcohol to provide low threshold/low barrier social work services within a "safety net medical home" ambulatory care setting. These services include, but are not limited to: psychosocial assessments, assisting patients in referrals to emergency and long-term housing stabilization, motivational interviewing, harm reduction counseling, referrals to residential treatment programs for substance use and mental health, mental health assessments and connection to care, and appropriate referrals to community agencies for financial benefits, legal, and immigration resources. Work with treatment team and inpatient/outpatient units to facilitate referrals and service.
The CSW I/II will utilize a professional knowledge of the principles and practices of social work to counsel individuals, couples, families, and groups to help them make optimal use of health care, social, counseling, or psychological services; to provide, within their recognized professional scope, psychotherapeutic services to patients/clients (which may include functioning as primary psychotherapist; to develop and improve the resources available to enhance social and personal functioning; to provide teaching and consultative services to other professionals and health care students; and to perform other related duties as required, services provided as part of a multidisciplinary team comprising the Family Health Center, Bridge Clinic.
The same professional concepts, principles and techniques are used by Clinical Social Workers regardless of the program in which they work, but the circumstances and details of assignments and the immediate occasions of service differ among the various University settings.
The individual may also participate in multi-disciplinary case conferences; in which care plans are developed for high-risk patients. The individual will be required to participate in social work meetings, staff meetings, case conferences, and other required clinic/departmental meetings. The individual may be responsible for providing culturally specific care to monolingual Spanish-speaking patient. The individual may facilitate weekly therapeutic groups. The CSW I/II will collaborate with substance use navigators and program manager on workflows for linkage to outside programs. The CSW I/II will have on site clinical supervision by a CSW with extensive experience with this patient population. The CSW I/II will be a role model for compassionate care for people who use drugs and alcohol.
The final salary and offer components are subject to additional approvals based on UC policy. Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement. The Bridge Clinic works to make substance use disorder treatment and safer use as accessible as possible for patients, including those referred from primary care, hospital/emergency department, or community settings. The Bridge Clinic is located within the Family Health Center primary care clinic and collocated with rapid access HIV/HCV treatment programs.
The Department of Family & Community Medicine (DFCM) is engaged in research, teaching, training, service and clinical activities located at seven primary UCSF and leased sites in San Francisco (Parnassus, ZSFG - Buildings 80, 90, 20 and the Main Hospital, Mission Center Building, Laurel Heights, Mission Hall, Lakeshore, Lakeside) as well as the OSHER center, Women's Health and the UCSF Primary Care Practices at China Basin and Laurel Village and at various local community, national and international research/service/education training and clinical sites. In addition to the Department's Family Medicine Residency Program located at ZSFG, the Department has affiliated residency programs in Fresno, Salinas, Santa Rosa, Kaiser Vallejo, Santa Rosa, and Contra Costa Health Systems. Authority and accountability for the Department reside with the Department Chair who delegates authority to Vice Chairs, the Executive Council, Directors and Chiefs of Service and their Administrators and the Department Associate Chair of Administration and Finance.
The mission of the Department of Family & Community Medicine (DFCM) is to educate students and residents in family medicine with an emphasis on meeting the needs of the economically disadvantaged and the medically underserved, advance knowledge in family and community medicine and develop methods of primary care that are effective, efficient, and accessible to all people.
DFCM is nationally recognized for our pioneering research programs in primary care and community health. The DFCM has a large teaching, research and clinical set of programs and activities based at Zuckerberg San Francisco General Hospital where this position is based. The Family Health Center (FHC) provides the full scope of primary care services for children, adolescents, adults, the elderly, and homebound patients. Services include continuity and urgent care for all ages, prenatal care and perinatal case management, family planning, HIV family clinic services, well child care, minor office procedures, mental health services, social services, substance abuse counseling, nutrition consultation, diabetes education and case management, nurse care management and more. Care is delivered within a primary care continuity provider model that optimizes use of health care resources. FHC patients reflect the rich ethnic and socioeconomic diversity of San Francisco. Required Qualifications CSW I:
- Master's degree in social work, psychology, or counseling from an accredited school of social work; or equivalent combination of education and experience; and knowledge and abilities essential to the successful performance of the duties assigned to the position
In addition to the above:
CSW II:
- Master's degree from an accredited school of social work, psychology or counseling and possession of the requirements necessary for application for licensure as a clinical social worker as determined by the California Board of Behavioral Science Examiners; or an equivalent combination of education and experience; and knowledge and abilities essential to the successful performance of the duties assigned to the position
- Ability to work cooperatively and communicate effectively with others. Ability to work in a high stress, high volume area and work well under pressure and with deadlines. Ability to follow oral and written instructions accurately, and be detailed oriented. Able to efficiently prepare written summaries of activities, set priorities and complete reports within required timeframes. Knowledge of medical and/or psychiatric terminology

- Must be computer-literate, working knowledge of EHR systems, and Microsoft Suite.
- Ability to handle demanding workload involving multiple tasks effectively and efficiently.

- Must demonstrate cultural humility and a strengths-based, recovery philosophy.
- Excellent motivational interviewing skills and compassionate approach to people who use drugs, including those who plan to continue using drugs and alcohol
- Demonstrated excellent attendance and reliability
Preferred Qualifications
- Prior related UC experience
- Case Management experience
- Possession of the requirements necessary for application for licensure as a clinical social worker as determined by the California Board of Behavioral Science Examiners; or equivalent
- Fluency in Spanish - ability to communicate effectively with monolingual patients
- Licensed Clinical Social Worker; or Licensed Marriage Family Therapist
- Social work experience providing services to people with substance use disorders that are diverse in gender, sexual orientation, language, country of origin, race, and ethnicity
