Essential Functions
>-Conduct comprehensive psychosocial assessments of patients to identify emotional, social, and behavioral factors that may impact their health and well-being.
-Evaluate patient support systems, living situations, and potential stressors.
-Collaborate with interdisciplinary healthcare teams to develop individualized care plans that address the psychosocial needs of patients.
-Coordinate and facilitate the implementation of care plans, ensuring continuity of care across various healthcare settings.
-Provide counseling and emotional support to patients and their families to address coping strategies, mental health concerns, and life adjustments related to their medical conditions.

Licensed Clinical Social Worker [LCSW - State License] - Generic - HR Only preferred Social Worker [Licensed Independent Clinical Social Worker - LICSW - State License] - Generic - HR Only preferred
Experience
Experience in clinical social work and care management in a healthcare setting. 5-7 years required and Management Experience 2-3 years preferred
Knowledge, Skills and Abilities
- Strong knowledge of psychosocial assessments, counseling techniques, and crisis intervention.
- Understanding of care coordination, discharge planning, and community resources.
- Excellent communication and interpersonal skills to work effectively with patients, families, and healthcare professionals.
- Empathy and compassion for patients and their families during challenging times.
- Ability to manage multiple cases and prioritize tasks effectively.
- Proficiency in using electronic health records (EHRs) and healthcare software.
