Summary
Responsible for providing comprehensive support to patients and their families, addressing psychosocial needs, and coordinating care to improve patient outcomes and overall well-being. Collaborates with healthcare teams, community resources, and other stakeholders to develop and implement care plans that promote holistic patient care and support. Yes Essential Functions >-Conduct comprehensive psychosocial assessments of patients to identify emotional, social, and behavioral factors that may impact their health and well-being. -Evaluate patient support systems, living situations, and potential stressors. -Collaborate with interdisciplinary healthcare teams to develop individualized care plans that address the psychosocial needs of patients. -Coordinate and facilitate the implementation of care plans, ensuring continuity of care across various healthcare settings. -Provide counseling and emotional support to patients and their families to address coping strategies, mental health concerns, and life adjustments related to their medical conditions. Education Master's Degree Social Work required Can this role accept experience in lieu of a degree? No Licenses and Credentials Licensed Clinical Social Worker [LCSW - State License] - Generic - HR Only preferred Social Worker [Licensed Independent Clinical Social Worker - LICSW - State License] - Generic - HR Only preferred Experience Experience in clinical social work and care management in a healthcare setting. 5-7 years required and Management Experience 2-3 years preferred Knowledge, Skills and Abilities - Strong knowledge of psychosocial assessments, counseling techniques, and crisis intervention. - Understanding of care coordination, discharge planning, and community resources. - Excellent communication and interpersonal skills to work effectively with patients, families, and healthcare professionals. - Empathy and compassion for patients and their families during challenging times. - Ability to manage multiple cases and prioritize tasks effectively. - Proficiency in using electronic health records (EHRs) and healthcare software.
