DESCRIPTION OF WORK

In this role, you will provide support to individuals transitioning back into the community following mental health hospitalization, as well as those at risk of premature hospitalization. Your efforts will aid consumers in their reintegration and help them maintain their place within the community, ultimately enhancing their recovery journey and fostering the self-confidence and independence necessary for successful living.

You will work in alignment with each consumer's Individual Service Plan (ISP) to implement specific goals and objectives, while also training them in home safety protocols. It will be essential to document observations, behaviors, and progress in a timely manner and to communicate relevant information with your Supervisor and multidisciplinary team members. Additionally, you may be required to transport consumers to appointments using a county vehicle when necessary

Your exceptional communication skills will be vital as you engage with consumers in their home environments, ensuring 24-hour support in a small facility catering to a limited number of individuals with serious mental illness (SMI).


Work Schedule and Additional Information:
  • This is part-time employment, 15 - 20 hour work week
  • Work hours are 8:00 AM to 4:00 PM, Wednesday, Thursday, and Saturday, with a 30-minute lunch.
  • Irregular hours may be necessary, including evenings, weekends, and holidays.
  • Employee may have occasional overnight travel or out of County travel for meetings or training.
  • Telework: You will not have the option to telework in this position.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • Six months of experience as a Residential Program Trainee (Commonwealth job title or equivalent Federal Government job title, as determined by the Office of Administration); or
  • One year of experience in a Human Services Agency working with the mentally disabled, the elderly, or children and youth; or
  • Two years of college which includes courses appropriate to the Human Services field; or
  • Any equivalent combination of experience and training.

Other Requirements:

  • You must be able to perform essential job functions.

Legal Requirements:

  • You must pass a background investigation.
  • A conditional offer of employment will require a drug screening.
  • This position falls under the provisions of the Child Protective Services Law.
    • Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
