Job Title: Community Outreach Navigator

Job Description:

The Community Outreach Navigator includes hands-on interaction with patients who have complex needs including but not limited to psychosocial, psychiatric, and/or substance use issues. This role works within a high-volume, high-energy department so the employee working within this environment must take initiative, be intuitive, creative, and can network and multitask.

Key responsibilities (are not limited to):

  • Ensures that Baptist Health Behavioral Health has established relationships with community partners, and thus, the individuals have an effective transition from community-based entities to the appropriate level(s) of care through planning, relaying of information, connecting and referring to appropriate agencies.
  • Visible in the community to appropriately network and engage with community partners and meet the needs of individuals.
  • Dually provide behavioral health consults to patients in the ER, SDS, ICU, OB and inpatient medical floors at Baptist Health Richmond on an as needed basis.
  • Provide a comprehensive psychiatric evaluation including suicide risk and gives recommendations on level(s) of care.
  • Staff cases with attending provider to collectively determine patient disposition. Makes regular contact with licensed supervisor to ensure accurate recommendations are made to the treatment team at Baptist Health Richmond.
  • Provides patients with resource information, connects them with the appropriate level of care providers and when appropriate coordinates direct admissions to inpatient psychiatric and detox units.
  • Perform levels of care evaluations and make recommendations.
  • While working with the population at large, must have specific foundational understanding of patient needs within mental health, substance use disorder and dual diagnosis population.

Requirements:

  • High school diploma and/or GED required.
  • College experience in Social Work or related field preferred.
  • Experience in mental health, substance use disorder or dual diagnosis settings preferred. Peer Support Certification preferred, or candidate willing to complete within one year of hire.
  • Minimum of 2 years' experience preferred. Preferably has experience with psychiatric and/or substance use disorder conditions.
