Position Summary The Neuroscience Center Nurse Navigator serves as a consistent care coordinator ensuring timely access and seamless coordination of services throughout the continuum of Neuroscience care. She/he will be an integral part of and professional member of the Interdisciplinary team promoting optimal patient care. The Navigator will provide medical information, education, and share knowledge regarding internal and external support services and resources. The Neuroscience Center Nurse Navigator will also be responsible for facilitating: rapid scheduling, coordination follow-up care, and tracking patient's to ensure completion of recommended treatment plan. Will identify and assist patient and families in eliminating potential barriers to care by identifying and referring to appropriate resources.Neuroscience,Stroke, andThrombectomy experience preferred! Scheduled Hours 8:00AM to 4:30PM Education Current Registered Professional Nurse in New Jersey. BSN required, master's degree in nursing preferred. Experience Minimum of 3-5 years of clinical nursing practice experience, primarily in area of specialty. Skills National certification, Orthopedic Nurse Certified (ONC), is preferred upon hire and required after one year of employment if not certified upon hire.
