Your role with us

A Clinical Nurse in Ambulatory Anticoagulation plays a critical role in assessing and managing patients receiving anticoagulation therapy by performing ongoing age-appropriate assessments, developing individualized care plans, and delivering interventions based on evidence-based practice. They ensure accurate documentation, patient education, and coordination with healthcare team members while maintaining safety and resource efficiency. Additionally, they demonstrate leadership in promoting quality care, supporting staff, and adapting to the dynamic needs of the ambulatory care environment.

What you'll do

  • Conducts thorough, ongoing assessments of patients' medical, social, cultural, and educational backgrounds, and evaluates patient/family readiness for learning to guide care.
  • Delivers patient-centered interventions based on individualized care plans, promotes self-care, involves family/caregivers, and responds effectively to emergencies.
  • Develops and revises holistic care plans reflecting patients' changing conditions, rehabilitation potential, and coordinates discharge or transfer plans with the healthcare team.
  • Maintains accurate and comprehensive documentation of assessments, interventions, education, and patient progress to ensure continuity and quality of care.
  • Communicates clearly and effectively with patients, families, and the healthcare team, sharing relevant information and anticipating patient needs.
  • Demonstrates leadership by applying evidence-based practice, supporting colleagues, managing resources, and assuming charge nurse responsibilities when needed.
  • Ensures a safe environment by following infection control policies, safety protocols, and proper use of equipment while managing resources efficiently to support smooth unit operations.
Required qualifications
  • Bachelor of Science in Nursing (BSN) or Non nursing Bachelor's Degree with MSN
  • Recent Basic Life Support Certification.
  • Current unencumbered licensure of practice nursing as a Registered Nurse in the state of Oregon.
  • 1 year of RN staff nurse experience required.

Additional Information

  • Acute care RN staff nurse experience preferred.
