Position Title Integrative Clinical Care Manager
Overview
St. John Fisher University is an independent, liberal arts institution that provides a transformative education to approximately 3,800 undergraduate, master's, and doctoral students each year. Across its five Schools, Fisher offers nearly 40 majors in the humanities, social sciences, natural sciences, business, education, and nursing; 11 pre-professional programs; and master's and doctoral programs in business, nursing, education, pharmacy, and public health. The University's residential campus boasts a robust living and learning community and a successful intercollegiate athletics program. Job Responsibilities
Case management (40%)

  • Assist students with obtaining information about purchasing health insurance, navigating questions about coverage and referrals to other resources (e.g., food, housing, finances) in the community.
  • Provide support and consultation for counseling staff requesting assistance with community referrals.
  • Maintain an updated comprehensive referral database for community mental health providers, including an account of current openings and low-fee or sliding scale fee options. Develop and maintain a list of resources for students on our website (e.g., self-help, community resources, podcasts).
  • Provide "bridging" services as needed while students connect to additional support(s).
  • Meet with students who may benefit from additional support and connections to progress academically.
  • When appropriate, refer students to additional internal and external supports. Assist with processes of verifying insurance benefits, scheduling intake with the community provider, and supporting the student in arranging logistics (transportation, copay) as needed.
  • Provide follow-up support to ensure a successful connection to the referral.
  • Consult with hospitals/higher levels of care regarding return to campus planning.
  • Create and maintain tracking and follow-up systems for student contacts.


Counseling/psychotherapy (35%)

  • By appointment, conduct individual (and/or group) counseling/psychotherapy sessions.
  • Be available for unscheduled walk-in contacts and/or direct crisis intervention.
  • Refer students to off-campus mental health providers when the treatment plan exceeds the parameters of care available through the Health and Wellness Center. Consult, refer and collaborate with other disciplines involved in the delivery of the student's mental health care.
  • Offer counseling interventions that may occur outside of standard counseling center business hours to meet student needs.
  • Maintain timely, accurate documentation of all clinical and clinical support activities.
  • Maintain confidentiality consistent with the ethical guidelines of professional organizations (e.g., ACA, APA, and NASW) and with New York State laws and regulations for mental health counselors.
  • Maintain appropriate records of client services as required by New York State laws, St. John Fisher, and currently accepted standards of professional practice; these include intake and assessment information, case notes, a termination summary, and results of any inventories/screenings.
  • Maintain professional skills and knowledge in mental health counseling consistent with current professional standards of practice (e.g., journal publications, professional development seminars, peer supervision, conferences, etc.)
  • Maintain certification and/or licensing as required by the State of New York, St. John Fisher University, and the counseling profession.


Outreach (15%)

  • Develop partnerships with agencies to help students access and utilize needed services (e.g., local mental health providers, agencies, and hospitals).
  • Build liaison relationships with campus partners to enhance HWC visibility and referrals.
  • Serve on the campus CARE team to provide early intervention support and case management feedback.

Education / Experience
Required: A master's degree and an active New York State professional license in mental health counseling, social work, or another related field.
Preferred: 5 + years of relevant professional experience, including work within a mental health setting, higher education, or other related field.
Competencies / Skills
The ICCC optimizes student satisfaction by providing accurate, efficient student support. This individual will hold a New York State license that allows them to provide counseling care. They will follow all of the required licensing activities to maintain an active NYS license. This position requires flexibility, as duties will vary based on student needs and changing volumes.

Work Environment

  • Office environment, limited access (by key).
  • Fast-paced office environment with over 3,000 student visits yearly.
  • Must be available to work extended hours, some late evenings, and some weekends.
  • Responsibilities outside of a normal business day will be required in this position.


Equipment to be Used
MS Office Suite (Excel, Word, Outlook, PowerPoint), Banner (data warehouse for all functional areas across campus), EHR software, fax, scanner, printer, copier, multi-line telephone.

Salary / Hourly Range $52,000.00 - $57,500.00 per year
