Care Navigation Supervisor

Hospice of Marion County, a member of Empath Health is currently seeking Care Navigation Supervisor to join our team.

The Care Navigation Supervisor:

• Supervises and leads a team of Care Navigators by providing oversight and coordination of their day-to-day work, assessing performance, and providing feedback to drive results.
• In addition, this individual will work directly with community partners outside of the Empath Health service area to ensure smooth facilitation of patient transfers/travelers to our service area and optimal access to the Empath Health Integrated Network of Care.

Qualifications/Requirements:

• Education: Associate's degree (A.A.) or equivalent from two-year college or technical school; bachelor's degree preferred
• Valid Florida license as a RN or LPN
• Strong preference for minimum two (2) years of hospice experience as RN or LPN
• Valid Florida State Driver's license
• Continuing education: As required for licensure
• Knowledge of hospice regulations such as admission/qualification criteria, reimbursement, and levels of care preferred.
• Excellent, positive relationship building, outreach efforts, problem solving, organizational, time management, creativity, interpersonal skills, written and verbal communication a must.
• Strong computer skills, including knowledge of Microsoft Office applications and Electronic Medical Record
