Home Health Intake Coordinator (Per Diem/On Call) - Hospice

Description

This is a per diem/on call position that may require working on weekends and with changing schedules.

The Intake Coordinator works closely in conjunction with patient/caregiver, physician, equipment distributors, infusion agencies, ancillary disciplines and community resources to provide coordinated quality patient care during the intake process. The Intake Coordinator must be able to handle data entry in different EMRs; use office related computer programs such as MS Word and Excel; make outreach calls to local facilities, hospitals, doctor's offices, and patient families and skillfully handle a difficult conversations over the phone with people from a variety of backgrounds; be able to think critically and flexibly to respond to a variety of situations.

Preferred qualifications:



  • 2 years of experience in a healthcare setting performing clerical tasks and processes.
  • Microsoft Office systems experience (Word, Excel, etc.)
  • Strong Phone Skills
  • Knowledge of community resources
  • Home Care Home Base experience
  • Bi-lingual Spanish/English speaking

About the Team

Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.
