Job Description: PROGRAM DESCRIPTION Assertive Community Treatment (ACT) is designed to engage persons with a Severe Mental Illness (SMI) whose needs are not met by traditional outpatient services. Services are delivered in the context and environment where they are needed (i.e. the team goes to the person). PURPOSE ESSENTIAL JOB FUNCTIONS 1. Directs day-to-day clinical operations of ACT team a. Provides direct supervision of team members b. Schedules staff work hours to ensure appropriate coverage c. Leads daily team and treatment planning meetings d. Evaluates status of all consumers and provides appropriate coordination of treatment to meet their challenging needs 2. Directs and coordinates consumer admission and assessment processes a. Schedules admission interview b. Supervises, directs, and coordinates completion of comprehensive assessment/reassessment of each consumer 3. Utilizes management tools to help ensure team's philosophy and goals are in accordance with the ACT model 4. Participates in staff recruitment, interviewing, hiring, orientation and performance plans 5. Oversees medical record documentation to ensure quality and accuracy 6. Provides on-call crisis interventions and back up according to team's policy 7. Participates in engaging and motivating team clients in all treatment goals while providing direct service 8. Manages ACT Team in maintaining organization's required productivity standards 9. Oversees completion of all required documentation in timely manner consistent with organization's guidelines. The intent of this job description is to provide a representative summary of the major duties and responsibilities performed by incumbents of this job. Incumbents may be requested to perform job-related tasks other than those specifically presented in this description. EDUCATION * Bachelor's Degree. * Master's degree preferred in Social Work, Psychology, Counseling or a related field. EXPERIENCE * Over six and up to eight years clinical experience with a supervisory background preferred, and experience with program management. * Thorough knowledge of relevant client population characteristics and related treatment planning. * Excellent oral and written communication skills. * Good interpersonal skills. * Good public speaking abilities. * Thorough knowledge of ACT model. * Good supervisory skills. PHYSICAL DEMANDS/REQUIREMENTS * Position requires lifting of 20 pounds maximum, with frequent lifting and/or carrying of items weighing up to 10 pounds, or requires significant periods of walking or standing. * Frequent exposure to threat of bodily harm from clients or families. * Normal tasks may involve exposure to blood, bodily fluids, or tissue. * May involve 24-hour on-call responsibilities.
