Behavioral Health Coach (LCSW or LMSW required)
We're seeking a compassionate professional to provide supportive coaching and care navigation services to clients with both medical and mental health conditions. This role focuses on empowering individuals to achieve their health and wellness goals through:

Essential Functions
  • Provide assessments, medical and behavioral health management interventions, coaching, education, training, and activities to coordinate care for HMP members.
  • Perform the assessment and evaluation of patients with behavioral health support and chronic disease by collecting medical, behavioral health, and social information to assist in the development of a patient-centered action plan.
  • Help patients and family to understand, accept and follow care plan
  • Provide counseling for members struggling with an active mental illness or mental disability as well as family members, patients, and staff
  • Collaborate with a team of clinical health care providers, and product technical experts providing education and information to members and making referrals for statewide community resources.
  • Collect and enter data and documentation in Telligen's care management system(s).
  • Participate in ongoing educational programs and special projects
  • Coordinate activities across the care continuum. This will include transitional care.
  • Providing motivational support and lifestyle coaching
  • Collaborating with Registered Nurses to reinforce health management strategies
  • Helping clients develop coping skills and healthy routines
  • Connecting clients to community resources
  • Offering guidance for daily living challenges
  • Supporting clients' overall wellness journey
This is NOT a clinical therapy position. Instead, you'll work as part of an integrated care team, focusing on:
  • Health behavior modification
  • Goal-setting support
  • Care coordination
  • Wellness education
  • Life skills coaching
Requirements
  • Licensed Clinical Social worker (LCSW or LMSW).
  • Skills and knowledge of human behavior and clinical aspects of medical Social Work.
  • Knowledge of techniques such as motivational interviewing, SMART goal setting, and assessing client readiness to change.
  • Excellent working knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditions.
  • Knowledge of local community resources.
  • Social Work background
  • Strong communication skills
  • Experience in supportive coaching
  • Knowledge of community resources
  • Passion for helping others achieve their wellness goals
  • This position is remote, with travel required to behavioral health clinics or hospitals as needed.
