1.00 FTE / 40 hours per week.



The BadgerCare Plus Coordinator serves as the designated HMO advocate and works with both members, Group Health Cooperative of South Central Wisconsin (GHC-SCW) staff and providers to assist members in navigating the health care system through outreach and connection to community resources. They assist other GHC-SCW departments as to the benefits and/or enrollment status of GHC-SCW members when issues of BadgerCare Plus coverage or eligibility arise. The BadgerCare Plus Coordinator will recommend and assist in the design of programs to improve various access issues, such as timely prenatal care, completion of well child exams and other clinical measures such as tobacco cessation, diabetes and asthma and addressing Social Determinants of Health (SDoH). They are responsible for helping to remove barriers to healthcare delivery for GHC-SCW BadgerCare Plus members through outreach.



They will work closely with Access Community Health Centers, UW Family Medicine Clinics, and school districts within the BadgerCare Plus service area to coordinate "School Based Services" and also represent GHC-SCW at community events and attend community meetings. They will also participate on GHC-SCW committees, as appropriate, and provide the perspective of the BadgerCare Plus population that GHC-SCW serves to other GHC-SCW committee members. The incumbent performs the job responsibilities with a health equity framework, independence in decision making and under the general guidance, direction, and supervision of the Population Health Manager.

Job Specific Minimum Qualifications



  1. Bachelor's degree from an accredited college or university in a relevant major such as Social Work, Healthcare, or equivalent combination of relevant education and experience is required.
  2. Minimum of two (2) to three (3) years of relevant clinical or community-based experience in healthcare delivery, social service or related setting is required. Experience as a community advocate, social worker or case manager is preferred.
  3. Possession of current State of Wisconsin Driver license with a good driving record, access to a vehicle, and the ability to travel is required.
  4. Ability to work with a diverse patient population is required.
  5. Ability to coordinate and project manage special projects and ongoing programming.
  6. Excellent customer service skills are required. Ability to be culturally sensitive and approach members and others in a non-intimidating and supportive manner, and the ability to represent GHC-SCW in a professional and compassionate manner is required.
  7. Knowledge of the BadgerCare Plus programs and the ability to learn GHC-SCW's services and benefits is required. Knowledge of community, state, and federal agency resources in order to assist the member and family achieve the highest possible level of functioning and service is required.
  8. Knowledge and experience in Social Determinants of Health (SDoH), health equity initiatives to remove health disparities, and health literacy and plain language best practices preferred.
  9. Knowledge of or ability to learn about general disease categories and socio-emotional impact, treatments, clinical care practices, pharmacological and medical terminology is required.
  10. Knowledge of or ability to learn about healthcare delivery systems including insurance and managed care. Ability to keep abreast of legal and regulation changes affecting the delivery of healthcare is required.
  11. Ability to function independently to meet organizational BadgerCare Plus goals is required.
  12. Knowledge of or ability to learn and use various computer programs including word processing, email, scheduling/calendar, and databases is required. Ability to use MS Office preferred.
  13. Ability to learn and use Epic is required.
  14. Ability to maintain confidentiality is required. Knowledge of or ability to learn HIPAA requirements.
  15. Excellent verbal and written communication skills and outstanding customer service skills are required.
  16. Excellent problem solving and organizational skills are required.
  17. Ability to listen, trouble-shoot, and be flexible is required.
  18. Ability to work efficiently and under stressful situations is required.
  19. Ability to work collaboratively and as a member of a team.
  20. Ability to work harmoniously and cooperatively with the public and all GHC-SCW staff is required.
  21. Evidence of the ability to understand and work closely with community contacts and resources is required.
  22. Ability to work a flexible schedule to meet customer needs is required. May occasionally include some weekends and evening hours for community events.
