Description
We are seeking a highly skilled and forward-thinking Nurse Practitioner (NP) or Physician Assistant (PA) to join our team in a dual-capacity role as our Quality Improvement Provider. This position combines direct patient care with strategic leadership in population health. The ideal candidate will be passionate about delivering high-quality, team-based care while leveraging data to identify trends, develop interventions, and support value-based care initiatives. Lifespark is a complete senior health company headquartered in St. Louis Park, Minnesota. Position: Quality Improvement Provider Wage Range: $118,000 - $130,000 annually, depending on experience Plus, mileage reimbursement of .70 cents per mile. Role type: Hybrid - working both remotely from home as well as on-site (corporate headquarters or at a senior living community) Schedule: 1.0 FTE, Monday - Friday 8:00am-5:00pm Lifespark Benefits Include:
