Description

The Children's Home of Poughkeepsie has been caring for children throughout the Hudson Valley for over 175 years! We strive toempower children and families to achieve optimal health and well being. We are currently seeking a Case Management Case Aide to join our UC Shelter Program.

Position Summary:

The Case Aide position is responsible for the collection and filing of documents that are essential to the family unification process. This position will provide support to the Case Management team and other key areas of the program, as needed.

Essential Functions and Responsibilities:



  • Provide assistance with the collection and filing of documents required for the family unification process
  • Assist a team of four Case Managers with filing responsibilities
  • Send email and schedule appointments for families on behalf of the Case Manager
  • Schedule Legal Orientation sessions for sponsors
  • Mail fingerprint cards or proof of address documentation, as needed
  • Complete address checks to verify the sponsor's address
  • Complete national and state sex offender registry checks on sponsors and family members, as needed
  • Assist in the collection of required documents and client signatures during the Initial intake process
  • Coordinate travel plans, including making flight arrangements for youth who are ready for discharge
  • Complete exit packet documentation to provide youth and families with resources after unification
  • Print and File required documents demonstrating services provided to youth in care for the case file
  • Complete Safety and Well Being follow up calls for youth who have been released from the program
  • Collect, upload and file documents completed post discharge
  • Ensure court documentation that arrives after the youth's discharge is communicated and filed accordingly
  • Attend all mandatory training as scheduled, including but not limited to New Hire Orientation, Sanctuary, TCI certifications, CPR/First Aid certifications. Must complete 40 hours annually of O.R.R (Office of Refugee Resettlement) training
  • Fulfill Mandated Reporter responsibilities according to NYS Justice Center and OCFS regulations.
  • Is familiar with and adheres to all agency policies and procedures as outlined in the Child Care Manual, Employee Handbook and Risk Management Manual.
  • Assists and performs general clerical duties for UC Program Director, Assistant Director and other staff as needed.



Schedule:

Monday - Friday 830am-530pm, Saturday-Sunday OFF

Qualifications

Minimum:



  • High School diploma or associates degree with clerical experience
  • Must be comfortable working with the public, in person and by phone.
  • Demonstrated clerical skills.
  • Demonstrated strong computer and word processing skills.



Preferred:



  • Associate's or Bachelor's degree in social work or a related field, bi-lingual Spanish- English
  • New York State Driver's License in good standing as determined by the Agency.
