Position Title: Senior LTSS Service Care Manager (RN)

Work Location: Member Facing to El Paso, may work CST and MST

Assignment Duration: 6 months (Temp to Hire)

Work Schedule: 8:00 am - 5:00 pm

Position Summary:

The RN position on the STAR Kids Assessment Team serves Medicaid recipients from birth through age 20 who have disabilities. This role involves conducting in-person assessments to evaluate members for initial evaluations, annual reassessments, changes in condition, and MDCP Interest List (IL) eligibility.

Key Responsibilities:




  • Conduct in-person assessments to evaluate Medicaid members for initial evaluations, annual reassessments, changes in condition, and MDCP Interest List eligibility.



  • Help determine the need for Personal Care Services (PCS), Private Duty Nursing (PDN), Durable Medical Equipment (DME), therapy, and the MDCP waiver program.



  • Complete documentation remotely from home after visits.



  • Cover members residing in El Paso and Hudspeth counties.



  • Complete 6 successful visits per week.



  • Submit assessments within 24 hours; finalize all other documentation within five business days.



  • Travel over 100 miles radius, including surrounding counties.



  • Conduct STAR Kids Screening and Assessment, Individual Service Plan, Emergency Disaster Plan, 1500 forms, Service Backup Plan, and Electronic Visit Verification.



  • Assess member's health status, resource use, treatment plans, goals, and provider options.



  • Follow up on requests for items/services and communicate with providers.



  • Ensure compliance with turnaround times; some visits may occur after 5 p.m. or occasional Saturdays.



  • Use two documentation days per week from home.



  • Balance clinical and holistic care including social, developmental, and environmental needs.

Qualification & Experience:




  • Required Education/Certification: Associate Degree in Nursing or Bachelor of Science in Nursing



  • Preferred Education/Certification: Bachelor of Science in Nursing



  • Licensure: RN required



  • Experience: Minimum 4 years RN experience



  • Must Haves:




    • Bilingual



    • Pediatric Experience



    • Minimum 4 years RN experience





  • Nice to Haves:




    • Private Duty Nursing



    • Home Health



    • School Nurse



    • Case Management





  • Disqualifiers:




    • Less than 4 years RN experience



    • Short-term roles across multiple organizations (excluding contract work)



    • Requesting time off within 10 weeks of start date
