DUTIES AND RESPONSIBILITIES:

* Act as a liaison between Eleonora Fagan Stabilization Center, Billie Holiday Center, Hope House Recovery Focused Shelter, Harbor Light Wellness Center, Pinehurst and Oakland Adult Rehabilitation Center (ARC)
* Works in tandem with case managers to develop the participant's individualized service and transition plan and goals
* Supports participants on strategies to execute both short and long-term goals, while identifying barriers to success
* Help participants access community resources such as permanent housing, employment, mental health services, treatment centers, and other support services
* Track participants' progress toward goals, provide feedback and work in collaboration with the assigned case manager and clinical supervisor to adjust recovery plans as necessary
* Offer tools and resources to help participants develop healthy coping mechanisms, build life skills, and engage in positive social activities
* Act as a liaison between participants and healthcare professionals, family members, and the broader recovery community
* Facilitate support groups, workshops, and meetings to promote community and peer support. Host office hours so participants can schedule time to discuss challenges and triggers
* Demonstrate an understanding of different backgrounds, providing services that are inclusive and respect the diversity of participants

* Work with the management to ensure compliance with all state, local, organizational, and funding regulations
* Ensure accurate record keeping, incident reports, and compliance with confidentiality regulations
* Foster relationships with community-based organizations, recovery service providers, and healthcare professionals to expand opportunities for participants and promote The Salvation Army's Recovery System of Care and Westside Community Services Treatment Care Network
* Represent the Elenora Fagan Stabilization Center at community meetings and forums to advocate for sober living programs and raise awareness of recovery issues
* Enhance partnerships for supportive services, such as job placement, educational opportunities, vocational training, and permanent housing
* Other related duties as required

EDUCATION AND EXPERIENCE:

* High school diploma or equivalent (required)
* Certified Alcohol and Drug Counselor (CADC, CCAAP, CAADE), or working toward it (required)
* CPR/First Aid Certification must be completed within 60 days of employment
* CA driver's license (preferred)
* Experience serving adults in recovery, or unhoused adults with substance use disorders and mental illness
* Experience working in a residential treatment program, therapeutic teaching community, or sober living facility
* Lived experience, as well as experience with reentry, addiction, and unhoused population (preferred)

SKILLS AND ABILITIES:

* Bilingual in Spanish (preferred)
* Ability to maintain confidentiality
* Ability to take initiative
* Great communication skills

EQUIPMENT USED:

* Modern Office Equipment and Relevant Software

PHYSICAL REQUIREMENTS:

* Ability to lift up to 25 lbs. (usually file boxes)
