Description

Nexus Family Healing is looking to hire a Independent Contractor Occupational Therapist to join our newest youth crisis and stabilization center set to open in fall 2025 in Hennepin County!

This new youth crisis and stabilization center is a 13-bed, short-term residential service is for Hennepin County youth with complex mental and behavioral health issues looking for appropriate care. The service will help to reduce the number of youth who are stuck in inappropriate care settings such as emergency rooms, police stations, juvenile detention or unstable home situations and provide treatment and therapeutic supports while triage, assessment, and transition planning take place with families.

Pay and Schedule:

  • Contracted Part Time Opportunity
  • 6 hours weekly onsite
  • Afternoon/evening and weekend schedule
  • Location: 1800 Chicago Avenue in Minneapolis
  • Pay: $100-150 per hour

Position Summary: Our Occupational Therapist will facilitate development and rehabilitation of patients with mental, emotional, and physical disabilities by planning and administering occupational therapy.

Primary responsibilities:

Assessment & Treatment Planning

  • Conduct timely, client-centered assessments to identify youth needs.
  • Interpret evaluations and test results to develop occupational therapy treatment plans in consultation with physicians or by prescription.
  • Establish and communicate care plans to the clinical team.
  • Monitor progress and adjust treatment plans as needed.

Collaboration & Communication

  • Work cooperatively with clinical, program, education, and nursing staff to gather comprehensive assessment data.
  • Educate and support program staff and families regarding youth care needs, including post-discharge recommendations.
  • Participate in discharge planning with relevant team members.

Documentation & Compliance

  • Complete and maintain all required documentation in accordance with Nexus standards.
  • Ensure accurate documentation of services provided, assessments, and discharge paperwork.
  • Attend team meetings and required trainings; support Nexus philosophy and treatment modalities.

Safety & Environment

  • Promote youth safety and maintain a positive, respectful, clean, and therapeutic environment.
  • Follow all Nexus safety policies and procedures across campus and during off-site activities.
  • Respond to emergencies using sound judgment and perform First Aid/CPR as needed.

Professional Conduct

  • Communicate clearly, professionally, and respectfully with internal and external stakeholders.
  • Maintain appropriate boundaries with youth and families.
  • Demonstrate problem-solving skills and adaptability in dynamic situations.
  • Perform other duties and projects as assigned.
Requirements

Required Education and Licensure:

  • Must meet the applicable State Board and Licensing requirements.
  • Valid driver's license required. Must meet state regulating agency and Home Office driving requirements

Preferred Education and Experience:

  • Experience in the Mental Health Industry preferred.
