Position Summary:
We have an exciting opportunity to join our team as a Program Supervisor - FHC (Spanish speaking preferred).

In this role, the successful candidate The Program Supervisor manages the day to day operations of the Ryan White Dental initiative funded
under the RW Part C services guidelines. S/he is responsible for the implementation of all activities including case management, and the assessment of patients, creating client care plans, facilitating case conferences, and collaborating with Dental Care Team members and Operational Leadership to establish sustainable pathways that ensure Oral Health treatment plan compliance and completion. The Program Supervisor ensures that interventions and documentation meet the quality standards set forth by the funder and the Family Health Centers and provides ongoing assessment and navigation services to patients as required by the grant. S/he collaborates with Ryan White Program Director to assess program quality and make changes to program and workflows as needed.

Job Responsibilities:

  • Oversee planning and implementation of the Comprehensive Plan that establishes sustainable linkages to care for PLWHA to Dental services across the Family Health Center network
  • Manages day-to-day operations of the program which includes providing direct assistance to patients and appropriate service provision and documentation.
  • Facilitates the interdisciplinary conversation with members of the HIV and Dental Care Team and provides oversight in terms of program compliance that ensures high quality of care meets applicable funderand internal standards.
  • Performs patient assessment, including initialsocial services, logistical and benefits assessment, to include Oral healthconsiderations. Performs regular re-assessments as specified by the grant
  • Regularly reviews documentation to ensure the qualityof program services and reporting, addresses any identified gaps in care ordocumentation
  • Collaborates with Operational Leadership increation and implementation of enhanced training materials includingasynchronous learning modules to be incorporated in Graduate Dental Educationcurriculum
  • Support treatment coordination activities utilizing electronic health records including Epic and Wisdom.
  • Contributes to Network-wide HIV quality efforts through participation in Viremia Rounds and HIV Quality Improvement meetings, as well as the support of the patient Community Advisory Board
  • Participate in program assessment activitiesthat include capturing and utilizing data to evaluate program success across a varietyof qualitative and quantitative measures.
  • Other duties as assigned.

Minimum Qualifications:
To qualify you must have a Bachelor's Degree required with 2 years of relevant supervisory experience, or the equivalent combination of education and experience. The successful candidate must also possess excellent interpersonal, verbal and written communication, and organizational skills. Knowledge of standard office software is essential, including proficiency with word-processing, spreadsheet and database software (e.g., Microsoft Office, preferred). Ability to multi-task and/or coordinate multiple assignments is required. Ability to interpret and implement policies and procedures, and effectively communicate detailed information to a diverse population, both verbally and in writing; excellent problem-solving skills is required.

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $64,350.00 - $64,350.00 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
