HealthPark Drive Fort Myers FL 33908 Department: Care Management Work Type: Full Time Shift: Shift 3/7:00:00 PM to 7:30:00 AM Minimum to Midpoint Pay Rate:$28.88 - $37.54 / hour SummaryThe Emergency Department Social Worker is experienced in rapid assessment, developing crisis management plans and aids physically ill patients and their families. The Social Worker assists in resolving the social and psychological problems related to the illness. The Social worker sensitizes other health care providers about the social and psychological aspects of illness. The Social Worker provides clinical coordination and therapeutic interventions through assessment, referral and interdisciplinary planning for high-risk populations with particular focus on frequent utilizers of the healthcare system. The Social Worker will link patients and families to systems that provide resources, services and opportunities to achieve and maintain maximum functioning. The Social Worker provides support to patients and families regarding adjustments to disease process and illnesses. The Social Worker also expedites linking patients with systems in the community to provide them with resources, services and opportunities to achieve and maintain maximum functioning, and conduct screenings for CBT resource linkage. Requirements Educational Requirements Degree/Diploma Obtained Program of Study Required/Preferred and/or Preferred and/or Minimum of 1-year clinical experience in medical or healthcare setting. State of Florida Licensure Requirements Licenses Required/Preferred and/or Communication and interpersonal skills, basic computer, analytical, grammar and spelling skills.
