The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.

Job Summary:

The APN will perform assessments of patient including history taking and physical examination prior to prescribing based upon the individual needs of patients in accordance with rights and privileges granted through RN licensure and APN certification

the nature and extent of assessments performed and resultant diagnoses made by APN's follow accepted standards of Advanced Nursing Practice

The APN will round daily with the collaborating physician and alternative with other APN in rotating cycle, Maintains knowledge of inpatient plan of care and responds to patients needs

Job Responsibilities:

  • Provides detailed transplant education to patient significant other
  • Collaborates with multi-disciplinary team and patient to coordinate discharge
  • Post-Transplant Outpatient- identifies and responds to recipient significant other physical, psychosocial and educational needs
  • Collaborates with other disciples financial counselor and social work when needed Reinforces education and follow-up requirements and compliance with medical regimen, including medications
  • Assures follow-up testing annual biopsies and visit schedules are organized and completed
  • Heart Failure VAD Participates in care of heart failure patients by performing assessments of patient including history and physicial examination and well as diagnoses and treatment
  • Performs lab review with coordinators, acts on results and adjust treatments as needed
  • Orders lab work, echocardiogram, CT scans and all necessary testing needed for the care of the specific patient
  • Adheres to and implements, in collaboration with cardiologist, established written protocols for medication titration in heart failure transplant patients
