Job Overview:

The Follow Up Specialist position provides support and behavioral health service linkage to individuals and families that are discharged from PESS.

Qualification

Required:

• Two years' experience in the mental health field required, along with a valid NJ
• Driver's license in good standing.

Scheduling Requirements (non-exempt positions):

• Full-Time
• Day
• Monday through Friday
• 8a-4p or 9a-5p

Essential Functions:

• They coordinate with the PESS team in providing in-person contacts on the PESS unit prior to discharge and telephone contacts within 72 hours to encourage compliance with discharge plans.
• They will conduct consumer follow-ups in the community, when appropriate, to decrease recidivism by facilitating wellness visits and encourage wellness and recovery.
• Provide documentation in the consumer's chart as necessary,
