Description

In this role, you will be primarily responsible for the clinical case management of patients with Inflammatory Bowel Disease (IBD) receiving biologic medication therapy. This includes overseeing all phases of the treatment process-from initial referral through ongoing patient follow-up.

Key Responsibilities

  • Provide advanced-level expertise and comprehensive understanding of IBD patients, therapy plans, and ancillary services to coordinate the patient's pathway to remission.
  • Coordinate interdisciplinary medical care delivery under the direction of the Medical Director.
  • Serve as a liaison among patients, families, medical staff, referring physicians, and insurance companies.
  • Manage complex care needs specific to IBD patients, including:

    • Transfusion requirements
    • Nutritional supplementation
    • Ostomy supplies
    • Coordination with surgical teams


  • Assist patients with documentation requests related to IBD care and disability.
  • Deliver individualized patient and family education.
  • Assess appropriate medical interventions to ensure effective resource utilization and quality assurance.


Additional Responsibilities

  • Collect data for clinical management and certification requirements.
  • Develop educational materials and programs for patients, nursing staff, and physicians.
  • Create clinical protocols, policies, and procedures.
  • Support community education and public relations initiatives.
  • Maintain and update the IBD patient registry by enrolling newly diagnosed patients.
  • Collaborate with research coordinators and clinic staff to enroll IBD patients in clinical trials.

Salary Range: $62.43 - $92.81/hourly
Qualifications

Required



  • Current California Registered Nurse license
  • Graduation from an accredited nursing program with a B.S. degree or Masters
  • BLS from the American Heart Association or American Red Cross
  • 5 years of relevant RN experience with a combination of critical care and Outpatient care.
  • An ability to case manage all aspects of an IBD patients' medical care from initial evaluation to plan of care adherence for proper remission status
  • Ability to apply a broad understanding and knowledge of medication therapy plans, immunology, immunosuppression, rejection and other complications of infusion biologic therapy plans.
  • Ability to communicate effectively with persons of various cultural backgrounds.
  • Good verbal and written communication skills
  • Ability to interface effectively with patients and their families
  • Excellent organizational skills, proficiency in meeting deadlines, ability to work independently and prioritize workload. Ability to manage numerous concurrent tasks in a high stress environment and deal with change
  • Ability to effectively coordinate and assess patient care and act as a liaison with nursing staff, outside agencies, team physicians, referring physicians, patients and families.
  • Ability to teach about IBD diagnosis and medical management issues.
