  • Ensure we meet regulations, performance standards, and quality expectations.

  • Build relationships with university programs and participate in hospital/community initiatives.

  • Develop and improve programs that support residents, families, and the care team.

  • Provide education and promote strong communication across staff, patients, families, and physicians.

  • Support student learning and possibly serve as a field instructor.

  • Deliver direct social work services when needed, including assessments, care planning, and admissions support.

  • Champion safety, quality improvement, and cultural diversity.

  • Maintain a positive, professional presence with everyone you interact with.

  • Stay current on compliance, regulations, and best practices.

Minimum Requirements

  • Master's degree in Social Work, Gerontology, or related healthcare field.

  • Graduate-level field instruction experience.

  • Five years of post-graduate social work experience in a healthcare setting.

  • Strong communication, organizational, and prioritization skills.
